At your first appointment, we will discuss your goals, learn more about each other and what brought you into our practice. Each client is different and no one mouth is the same. We will evaluate and assess the best treatment for you.
After we review your case thoroughly, we will be able to create a customized plan of action for your dental health needs and aesthetic goals.
First visit checklist :
- X-Rays (if applicable): You can either request these from your previous dentist or bring them with you if you have on hand. Any files can be emailed to the office
- Filled out any registration forms: New Patient Forms, Consent Form, Office Policies, Privacy Policy. Bringing these four forms to your appointment it will help us expedite your check-in
- Insurance information: If you have dental insurance, please bring your card and any necessary completed forms, including referral paperwork (if applicable)
PLEASE NOTE: Patients under 18 years of age must be accompanied by a parent or guardian on their first consultation visit.
